2. Fill up all the required information in the form accurately. After completing all fields, click the submit button. Please proofread all required data, including the correctness of your email address.

4. After payment, take a screenshot or scan your Gcash/Paymaya receipts (make sure that the reference number, recipient's name, account number, amount and date are visible in your screenshot). Bank deposit slips should indicate the NAME OF DEPOSITOR. This serves as your "PROOF OF PAYMENT."

5. To validate your payment, email your proof of payment to finance.sirpedz@gmail.com. Your email should include the following information: Full name, Amount paid, Transaction Date, Reference Number, Enrolled Schedule No. (e.g. - FACE TO FACE - CEBU), Special Discount if any (e.g. - D1), and Purpose of Payment.

6. Once we receive and validate your proof of payment, we will email your ACKNOWLEDGEMENT NOTICE, which confirms your official enrollment in our review center, including the amount paid and enrolled schedule.
